New Project (Initial Review) Email Submission Process

Please submit your protocol and a cover letter (using the template below) to the OHRA general inbox - ohra_uhg@uhg.com

To Amend or Modify an Existing Project:

All amendments to existing research (for R&D and non-R&D projects) should be submitted via email to the OHRA general inbox - ohra_uhg@uhg.com. Your email submission must include:

  • Cover letter with:
    • A brief explanation of the purpose of the amendment and any pertinent information such as deadlines, internal requirements, other corporate entities that have reviewed these materials etc…
    • A summary of all changes/additions being made and rationale for each change (rationale = why this is being changed and the impact of the change on the protocol)
    • A complete list of documents being submitted for review (all documents require a version and date)
  • Completed amendment form (download below)
  • Tracked copies of updated documents demonstrating all changes
  • Clean copies of any new documents being submitted for the first time
  • Versions and dates on all documents for tracking

After initial approval of your project, subsequent submissions to change, renew, update or close your project require a completed application form. Click to download and complete the appropriate form prior to creating your submission in the system. These forms require use of Microsoft Word.
